Our Parent and Child form fully supports events that are for youth attendees who are accompanied by an adult. The form can handle events like Father-Son camping trips, Mother-Daughter sporting activities, Mother-Son Retreats or Father-Daughter dinner dances while allowing you to register up to 4 children at a time! The form also comes with an online waiver that accepts e-signatures and is fully compliant with all legal and organizational requirements.

Once you receive your form, you will be able to modify the different options yourself and clone your event form when you want to run a similar event again at another time.
